Welcome to the Siloam Springs School District !

To register your new student, please complete the following steps:

  •  Complete the online registration form using the link below

  • Bring the following documents to your student’s school on or before August 3, 2020.

    • Documentation of your child's birth date (any one of the following)

      • Child's Birth Certificate

      • Hospital Record

      • Statement by the Local Registrar or County Recorder

      • Attested Baptism Certificate

      • Affidavit of Date, Place of Birth by Parents or Guardians

      • United States Military Identification

    • Proof of Residency

    • Immunization record

    • Social Security Card (parents may request an alternate number assigned by the district)

  • Parents/guardians will also be given the following forms to complete

    • How to Apply For Free and Reduced Price School Meals 

    • Chromebook Agreement Form                                               

    • Home Language Usage Survey (Initial Enrollment ONLY)

    • Transportation Policies

    • Receipt of Student Policy Manual (may read digitally)

    • Medical Information Form

For Kindergarten Students

  1.  Students must be 5 on or before August 1, 2020 to register for Kindergarten

  2. Current Physical/Wellness Exam is required within the first 90 days of school

McKinney-Vento: Temporary Residence Form

If your family lives in a temporary residence, you should complete one of the forms (either English or Spanish) below.  See McKinney-Vento information located on the Parents website for more information.

Pre-K Application

Northside Elementary is now taking applications for Preschool students for the 2020-2021 school year.  All applications will be submitted online and can be accessed below. An additional Arkansas Better Chance application will also need to be completed. This document may be picked up from the Northside office Monday - Friday during hours of 8am - 3PM.

Your child must be 3 or 4 years old on or before August 1, 2020 to apply for preschool. Space is limited and will be filled on a first come, first qualified basis. All qualified children who are four years old by this date will be notified if accepted.  All qualified three-year-olds will be added to the waiting list.

In order to qualify you must:

·        Meet income guidelines

·        Complete online application

·        Provide necessary documentation

In order for applications to be complete follow these steps:

Complete the online registration form through this link

Bring the following documents to Northside Elementary :

  • Proof of Birth
  • Proof of residency
  • Immunization record 
  • Social Security Card - parent may request a number assigned by the district if preferred
  • Health Screening (Wellness Exam)
  • Income Verification (last 30 days of every adult living in the home, tax return, etc.)

The additional documents can be brought to the Northside office Monday- Friday between 9:00 a.m. and 2:00 p.m.

8 - $88,240

For each additional person add $4,420 per person.   200% of Federal Poverty
